Each account (User ID and Password) is specific to one program code. If you have both an LPN and an RN program, you would have two separate accounts.
You will not be able to submit a graduate through the portal without a SSN. You should not enter a ‘dummy’ SSN in order to proceed (such as 999999999).
If you have a student without a SSN, you will need to send the traditional school lists, as you have done in the past (provided their graduation date is within the last year). If the graduation date is more than a year in the past, you would submit an official transcript.
Notify us immediately at MQA.Nursing@flhealth.gov; be sure to provide the program code they were submitted under, the date you submitted them, and their name.
Since this process includes an auto-approval feature, it is imperative only individuals who have completed all requirements are submitted. Repeated issues with incorrect data submissions will result in portal access being disabled, and you would only be authorized to submit the traditional school lists or official transcripts.
At this time, only LPN and RN programs have the capability to submit their graduates via the portal.
Only programs in Approved, Accredited or Probationary status will be able to submit via the portal.
If your program’s status is changed to voluntarily closed or terminated, you would follow the former procedure: submit the traditional Word document school lists, or if the graduation date is more than a year prior, an official transcript.
You would need to provide an official transcript; either by US mail, or through a secure third party (Parchment, etc.).
The school list information will not load if the name, DOB and SSN do not match exactly. If you become aware that you have submitted a student with a typo in the name, DOB, SSN, you will need to resubmit them via the portal with the correct information. As long as the new submission matches their record in our system, it should then load.
It would depend on where the information is incorrect. The name, DOB and SSN must match exactly.
If you have verified that the DOB and SSN you have submitted are correct, then the applicant likely entered one of these incorrectly on their application. They will need to provide their Driver’s License/Social Security Card to have the information corrected and then the education should load. The applicant should be listing the name on the application exactly as it appears on the ID they will present at the test site- so that is also how you would need to submit their information via the NPP.
If you determine that you have submitted a student with a typo in the name, DOB, SSN, you will need to resubmit them via the portal with the correct information. As long as the new submission matches their record in our system, it should then load.
You will need to review your CSV file to ensure you have the correct degree, the SSNs and DOBs are all in the correct format, and that you do not have any commas in the name fields (Ex. Smith, Jr). Suffixes will not import – you will receive an error. It is separate from the last name field in our system, so you would not include in the name field. The name field also has a character limit of 16. If you have verified the above, and continue to receive errors, please contact us at MQA.Nursing@flhealth.gov.
You will need to contact us at MQA.Nursing@flhealth.gov when you are ready to submit your first cohort of graduates and we will provide the User ID and Password at that time.
Can I qualify for licensure by endorsement under this clause – “Has actively practiced nursing in another state, jurisdiction, or territory of the United States for 2 of the preceding 3 years without having his or her license acted against by the licensing authority of any jurisdiction”?
No. That provision in section 464.009, Florida Statutes, was removed on October 9, 2024. Applicants for licensure by endorsement must now meet the requirements of the MOBILE endorsement pathway.
You will need access to a desktop or laptop computer with a compatible web browser (Internet Explorer, Mozilla Firefox, or Google Chrome) installed, your social security number and date of birth.
If you are applying for a license and have not created an account, please follow these steps:
- Visit https://mqa-vo.doh.state.fl.us/datamart/voservicesportal, click on ‘Create New Applicant Account. Once on the registration screen, follow the prompts for new user registration using a valid, active email address.
- Check your email to locate your new account user ID and temporary password. Using your temporary password and user ID provided via email, log into your account. (Note: You will be prompted to create a new password for your account. Enter the temporary password in the “Old/Temporary Password” field.)
- Once logged in, you will be prompted to add any existing health care licenses or previously submitted applications to your account. Follow the prompts to add your license or application using your social security number, date of birth, and mailing address zip code. If you do not have a current license or pending application, or your license is null and void, select “No” to proceed with applying for a license.
If you are already licensed please follow these steps:
- Go to flhealthsource.gov and click the ”Account Login” button.
- Enter your Personal Identifying Information (PII), then click the Login button.
- Confirm or update your MQA Services Account email address, then click the “Continue” button.
- You are now in the Dashboard. During renewals, a “Renew My License” option will populate no later than 90 days prior to your license expiration date. (Note: If you do not see the “Renew My License” option, please check back at a later time.)
*** If the “PII Failed” screen displays, click the ”Other Login Option” button and enter your existing User ID and password to access your account.***
